Physical Therapist (PT) – Facility & Community | Hightstown, NJ | PRN with Part-Time Growth Opportunity
Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to help establish and grow our therapy services at a facility in Hightstown, NJ.
This position will begin as a PRN facility-based role, providing one-on-one physical therapy services to an established patient population at a single location.
As the caseload grows, we plan to introduce community-based patients in Hightstown and surrounding areas, creating the opportunity for this position to develop into a Part-Time role.
This is an excellent opportunity for a Physical Therapist who wants to get involved in a growing territory early and potentially expand their caseload over time.
What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you'll initially evaluate and treat patients at a single facility in Hightstown.
As community referrals are introduced, you'll have the opportunity to expand your caseload by treating patients in their homes throughout the surrounding area.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments.
- Help patients improve strength, balance, mobility, endurance, and functional independence.
- Provide gait training, transfer training, therapeutic exercise, and fall-prevention interventions.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Recommend assistive devices and adaptive equipment when appropriate.
- Educate patients and caregivers on exercises, mobility strategies, and safety.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, facility staff, families, and interdisciplinary care team members.
- Coordinate with Stern's administrative team regarding scheduling and patient care.
Compensation:
PRN 1099 – Minimum 5 Patients:
- $72 per follow-up visit in the community and $60 per visit in the facility.
- $85 per evaluation
- Fixed per-visit compensation
- PRN positions are 1099 independent contractor roles only
Potential Part-Time Opportunity:
As the Hightstown territory grows and we add community patients, you may have the opportunity to transition into a Part-Time position.
Part-Time – Minimum 10 Patients:
W-2
- $65 per follow-up visit in the community and $55 in the facility.
- $75 per 4-unit evaluation.
Growth Opportunity:
This position is designed to grow alongside our Hightstown patient population.
You'll initially have the convenience of treating patients at one facility, providing a centralized and consistent starting caseload. As Stern develops additional referrals in the surrounding community, you'll have the option to take on community patients and increase your overall volume.
This provides the opportunity to progress from:
PRN Facility Caseload → Facility + Community Caseload → Part-Time Opportunity.
Part-Time W-2 Benefits:
If the position develops into a Part-Time W-2 role, eligible benefits include:

Qualifications:
- Active Physical Therapist license in New Jersey required.
- Graduate of an accredited Physical Therapy program.
- Reliable transportation required as community patients are added.
- Strong clinical, communication, documentation, and time-management skills.
- Ability to work independently and collaborate with facility staff and the interdisciplinary care team.
- Facility, home health, or geriatric experience preferred but not required.
