Palliative Care Nurse Practitioner

We are seeking a compassionate and experienced Nurse Practitioner (NP) to join our nonprofit, community-based palliative care program. This is an opportunity to provide specialized, patient-centered care to individuals living with serious and complex illnesses while helping patients and families navigate symptoms, treatment decisions, and goals for care.

Our palliative care team partners with patients, families, primary care providers, specialists, and other healthcare professionals to improve quality of life and ensure that care reflects what matters most to each patient.

Position Overview

The Community Palliative Care Nurse Practitioner provides palliative care consultation and ongoing support for patients living with serious illness in the community. The NP will conduct comprehensive assessments, develop individualized recommendations for symptom management, facilitate advance care planning, and collaborate closely with each patient's existing healthcare providers.

This role is ideal for an NP who values relationship-centered care, interdisciplinary collaboration, and the opportunity to spend meaningful time addressing the physical, emotional, and practical needs associated with serious illness.

Key Responsibilities

• Conduct comprehensive palliative care assessments for patients living with serious and complex illnesses.
• Evaluate and provide recommendations for management of pain and other distressing symptoms, including dyspnea, nausea, fatigue, anxiety, constipation, sleep concerns, and decreased appetite.
• Develop individualized palliative care plans based on each patient's clinical needs, priorities, values, and goals.
• Facilitate  Facilitate goals-of-care and advance care planning conversations with patients and families.
• Provide education regarding advance directives and assist patients in identifying and documenting their healthcare preferences.
• Collaborate and communicate with primary care providers, specialists, and other members of the patient's healthcare team to promote coordinated, patient-centered care.
• Provide education and support to patients and caregivers about symptom management, disease progression, available resources, and care options.
• Identify changing patient needs and help facilitate appropriate services and levels of support.
• Participate as an integral member of an interdisciplinary palliative care team.
• Complete timely, accurate clinical documentation and communicate recommendations to referring and collaborating providers.

Requirements

 Qualifications

• Current Nurse Practitioner licensure and appropriate prescribing authority.
• Graduate degree from an accredited Nurse Practitioner program.
• Current national NP certification.
• Experience in palliative care, hospice, geriatrics, oncology, primary care, complex chronic illness, or a related specialty strongly preferred.
• Strong clinical assessment and symptom-management skills.
• Experience with or demonstrated interest in serious-illness communication, advance care planning, and goals-of-care discussions.
• Excellent communication and interpersonal skills with patients, families, and healthcare professionals.
• Ability to work independently while functioning collaboratively within an interdisciplinary team.
• Commitment to compassionate, culturally responsive, patient- and family-centered care.
• Valid driver's license and reliable transportation for community-based visits, if applicable.

 Schedule & Compensation

• 32–40  hours per week
• Salaried $91,520- $147,680/annual
• Competitive salary commensurate with experience and qualifications
• Benefits available based on scheduled hours and organizational policy
• Primarily community-based palliative care practice
