Job Overview
Maple Grove Senior Living is seeking an experienced Rehabilitation Services Manager to lead our therapy program and support exceptional outcomes for residents receiving skilled rehabilitation and long-term care services.
The Rehabilitation Services Manager provides leadership and oversight to our physical, occupational, and speech therapy teams while ensuring high-quality resident care, regulatory compliance, strong interdisciplinary collaboration, and effective department operations.

Key Responsibilities
- Lead and support Physical Therapy, Occupational Therapy, and Speech Therapy services.
- Oversee daily rehabilitation operations, staffing, scheduling, and workflow.
- Promote high-quality, resident-centered rehabilitation and positive clinical outcomes.
- Monitor therapy performance, documentation, productivity, and regulatory compliance.
- Collaborate with nursing, physicians, administration, and the interdisciplinary team to coordinate resident care.
- Recruit, onboard, develop, and retain rehabilitation team members.
- Monitor department financial performance and manage resources effectively.
- Identify opportunities to strengthen rehabilitation services, census, and program performance.
- Maintain compliance with applicable federal, state, professional, and facility requirements.

Requirements
Requirements
- Degree from an accredited Physical Therapy, Occupational Therapy, or Speech-Language Pathology program preferred, with applicable active Kentucky licensure in good standing.
- Previous rehabilitation leadership or management experience strongly preferred.
- Skilled nursing, long-term care, or post-acute rehabilitation experience preferred.
- Strong understanding of rehabilitation operations, documentation, reimbursement, and regulatory requirements.
- Demonstrated leadership, communication, financial management, and problem-solving skills.
- Ability to develop strong relationships with residents, families, staff, and healthcare partners.
- Ability to meet the physical requirements of the position, including prolonged standing/walking and assisting with lifting or transferring up to 50 pounds as needed.
