Director – Medication Adherence & Access Team (Job Overview)
Schedule: Monday-Friday (8AM-5PM)

As a Director – Medication Adherence & Access Team, you will be an essential part of a collaborative team that focuses on improving medication adherence and access for our patients.
In this role, you will:
- Provide oversight to programs that enhance medication adherence and access.
- Collaborate with a compassionate team to support patients in managing their medication regimens.
- Engage in impactful activities, such as data collection, communication with healthcare providers, and outreach to at-risk clients.

Your work will directly contribute to transforming healthcare outcomes for individuals with chronic conditions, empowering them to lead healthier, fulfilling lives.
Key Responsibilities
- Support the development of programs designed to enhance medication adherence.
- Oversee and conduct investigations to minimize delays in patient medication needs.
- Monitor prescription renewal requests to ensure timely service delivery.
- Identify opportunities for programmatic improvement and recommend solutions.
- Communicate effectively with Legacy leaders and providers to foster inclusiveness and program success.
- Research Pharma Patient Assistance Programs and provide educational resources for team success.
- Evaluate innovative approaches to ensure accessibility for all Legacy patients.
- Participate in Medication Therapy Management for patients with chronic diseases.
- Assist in training pharmacists, providers, and personnel.
- Standardize patient activities and evaluate the impact of interventions.
- Engage in quality improvement efforts addressing medication barriers.
- Develop educational programs for patients and healthcare personnel.
- Display friendliness, compassion, and professionalism in all patient interactions.
- Show adaptability and take initiative to support colleagues and the mission of Legacy Community Health.
Minimum Qualifications
- Graduate of an accredited college/university of Pharmacy (Pharm.D or B.S. with equivalent experience)
- Current Texas Pharmacist licensure in good standing (or eligible)
- Certification in immunization delivery, smoking cessation, and naloxone
- Post-graduate pharmacy residency required
- 5+ years of healthcare experience required
- 1+ years of community pharmacy experience preferred
- 3+ years of medication adherence counseling experience preferred
- 1+ years of Medicare market experience preferred
- Experience in HIV/AIDs, Geriatrics, and/or Pediatrics, preferred
- Computer experience Windows, Excel, and Accounting software.
- Must be able to manage multiple deadlines and decision making responsibilities
- Adept in decision making responsibilities
