Honor Community Health is a 501c3 Federally Qualified Health Center co-located within 20 locations. Our mission is to provide for the health and wellness needs of the underserved of Oakland County through the provision of comprehensive, integrated primary, behavioral health, and dental care. Position Description
The Director of Senior Services provides strategic and operational leadership for geriatric-focused sites, services, and programs that support older adults, caregivers, and families across Honor Community Health’s service area. The Director leads the planning, development, implementation, management, and evaluation of senior-focused service models within existing health centers and in community-based locations where older adults can more easily access care and supportive services. This role uses human-centered design principles to understand the needs, preferences, barriers, and lived experiences of older adults, caregivers, staff, and community partners; designs accessible, person-centered services; establishes workflows and operational infrastructure; and builds coordinated partnerships with healthcare providers, social-service agencies, municipalities, funders, housing partners, and community organizations. The Director is accountable for program quality, regulatory and contractual compliance, fiscal stewardship, workforce leadership, performance measurement, continuous improvement, and a consistently positive patient experience. The Director advances healthy aging, independence, social connection, equitable access, and successful navigation of health and supportive services while ensuring that all work aligns with Honor Community Health’s mission, policies, and standards.
 

Our standard hours are Mon-Fri, 8:30am-5:00pm.

What are we looking for?

• Bachelor’s degree in gerontology, social work, public health, healthcare administration, human services, nonprofit management, or a related field required; master’s degree preferred.
• Minimum five years of progressively responsible experience in aging services, community health, human services, social services, or a related setting, including at least three years of supervisory or program-management experience.
• Demonstrated experience developing, implementing, and evaluating programs for older adults, caregivers, or medically underserved communities.
• Knowledge of aging-services systems, community-based supports, benefits and resource navigation, social determinants of health, healthy aging, caregiver supports, and barriers affecting older adults.
• Demonstrated ability to use human-centered design, patient-experience feedback, process improvement, and data analysis to develop, test, refine, and sustain programs and services.
• Working knowledge of applicable federal, state, and local requirements, privacy practices, contractual obligations, funding requirements, and ethical standards.
• Demonstrated skill in strategic planning, budgeting, grant or contract management, data analysis, performance reporting, quality improvement, and risk management.
• Strong leadership, staff-development, interpersonal, facilitation, public-speaking, written-communication, negotiation, and conflict-resolution skills.
• Ability to build productive relationships with older adults, caregivers, staff, executives, healthcare providers, funders, public officials, and community partners.
• Proficiency with Microsoft Office, electronic records or case-management systems, virtual-meeting platforms, and data-reporting tools.
