Care Coordinator (RN) - Primary Care Clinic - Full time

We are currently hiring for a full time Care Coordinator (RN) within the Primary Care Clinic department here at Aspen Valley Health.

Work Shift:

Days - 8 Hour

Compensation Range:

$43.19 - $69.10 Eligible new hires may qualify for a sign on bonus up to $10,000.

The compensation range listed is an estimate. Final compensation will be determined based on your qualifications including years of experience performing similar work, level of formal education in a relevant field of study, and any professional certifications related to the position.

Requirements:

Minimum Qualifications & Requirements

• At least two years of experience in a clinical setting as a Registered Nurse, Advanced Practice Provider (NP/PA), Clinical Social Worker, or similar.
• Unrestricted license to practice as a Registered Nurse (RN) in the State of Colorado, or ability to obtain prior to start date
• Basic Life Support (BLS). May be obtained upon hire.

Preferred Qualifications

• Bachelor's (BSN) or Master's (MSN) in Nursing preferred.
• Care coordination or case management experience preferred.
• Experience with EPIC preferred.
• Bilingual in Spanish preferred.

Position Overview:

The Care Coordinator uses ateam-based approach focused on a continuous partnership between the patient,family, caregiver, physician, clinicians, hospital and community resources toprovide coordinated care for complex patients. The Care Coordinator is responsible for direct and indirect patient careacross the care continuum, with an emphasis on assisting high-risk patients inthe community, including Medicare, Medicaid, VHA and other payor mixbeneficiaries. Activities will promote,maintain and improve the health of patients by creating and maintaining acentralized, multidisciplinary care plan and will provide social support,advocate for individuals, address community health needs and perform communityoutreach such as health screenings.
