What You'll Do
- Provide skilled physical therapy services under Medicare Part B to residents in an assisted living community.
- Complete comprehensive physical therapy evaluations and assessments of mobility, strength, balance, coordination, gait, and functional abilities.
- Develop individualized treatment plans based on each patient's needs, goals, and level of function.
- Provide therapeutic interventions designed to improve strength, mobility, balance, endurance, coordination, and overall functional independence.
- Evaluate patients' progress and modify treatment plans as appropriate to achieve optimal outcomes.
- Help patients safely perform activities of daily living and maintain their independence within the community.
- Identify fall risks and functional limitations and implement appropriate interventions to improve safety and mobility.
- Educate patients, families, caregivers, and facility staff on exercises, mobility strategies, fall prevention, and recommendations for continued progress.
- Coordinate care with physicians, nurses, occupational and speech therapists, facility staff, caregivers, and other healthcare professionals.
- Communicate changes in patient condition and progress toward goals with the appropriate members of the care team.
- Participate in interdisciplinary care planning and collaborate with the assisted living community to promote continuity and quality of care.
- Maintain accurate and timely clinical documentation, including evaluations, treatment notes, progress reports, plans of care, and care coordination.
- Demonstrate strong customer service and build positive relationships with patients, families, facility staff, and healthcare partners.
- Participate in ongoing education and professional development to maintain clinical knowledge and provide high-quality care.
- Be self-motivated, growth-minded, and committed to delivering an exceptional patient experience.
What You'll Need
- Active Florida Physical Therapist (PT) license
- Graduate of an accredited Physical Therapy program
- Strong clinical assessment and treatment skills
- Knowledge of evidence-based physical therapy practices
- Strong communication, organization, and critical-thinking skills
- Ability to work independently while collaborating effectively with a multidisciplinary team
- Commitment to compassionate, patient-centered care
- Excellent customer service and interpersonal skills
- Self-motivated and committed to continuous professional growth
- Experience with older adults, geriatrics, outpatient therapy, assisted living, skilled nursing, or home health is a plus
