Schedule Details/Additional Information:

Care Coordinator RN works closely with the physicians and APPs. Their role is to support their clinician and their patients with a strong focus on oral chemotherapy adherence while supporting patients navigating through any barriers to care with both internal and community resources throughout their oncology/hematology journey. The Care Coordinator primarily works in the Crystal Lake location however will also follow their clinician to the Barrington Office on the Good Shepherd Hospital campus one day per week. Schedule is Monday through Friday; no weekends or holidays. Office is in operation from 7:30am to 5pm. Staggered starts occurring to equal an 8.5-hour day which includes a 30-minute mealtime. Lab and Pharmacy are on-site. Onboarding with a preceptor is very individualized. All training is done on-site. 

Pay Range:

$38.20 - $57.30

Job Description Summary: 

Navigates patient and family members through the diagnosis, treatment, and follow-up of the patient throughout the continuum of care ensuring smooth transitions between care settings. Collaborates with a multi-disciplinary team to support patients and families in addressing the following dimensions of care: education, psychosocial needs, advocacy, and continuity of care.

We’re Looking For: 

  • Registered Nurse license issued by the state in which the team member practices 

  • Bachelor's Degree in Nursing. 

  • Typically requires 5 years of experience in nursing that includes performing nursing assessments, care plan development, and educating patients.    

 

What You’ll Do: 

Monday through Friday; no weekends or holidays. Office is in operation from 7:30am to 5pm. Staggered starts occurring to equal an 8.5-hour day which includes a 30-minute mealtime

  • Provides continuity of care by ensuring smooth transitions between care settings Develops a relationship with patient and their multidisciplinary team to facilitate and/or navigate through subsequent treatment and follow-up to reflect continuity of care. 

  • Completes a nursing assessment to identify the patient's individualized needs and collaborates with multidisciplinary team and other departments to develop an interdisciplinary plan of care. 

  • Supports the patient and family by providing education, addressing psychosocial needs, advocating, providing continuity of care and identifying barriers to care Navigates patients and family members throughout the diagnosis, treatment and follow-up of the patient throughout the continuum of care. 

  • Provides expert clinical advice to the multidisciplinary team in designing collaborative teaching plans/programs and in planning/providing patient/family education and discharge planning including wellness and health maintenance within designated specialty. 

  • Coordinates timely, seamless, evidenced based care across the care continuum resulting in best patient outcomes.

Physical Requirements and Working Conditions 

  • Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.  

  • Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.  

  • Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.  

  • Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.  

  • Maneuver foot pedals on carts or machines.  

  • Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.  

  • May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease. Therefore, protective clothing and equipment must be worn as necessary.  

  • Must be able to respond quickly to changes in patient and/or unit conditions.  

  • Physical Abilities Testing may be required 

  • Additional department specific physical requirements may be identified for unique responsibilities within the department by the nurse leader.
