SUMMARY: Provides telephonic triage and document review services that require clinical judgement and interpretation and acts as a liaison between the patient, care team(s), and their primary care provider. The core Success Factors include: Instill Trust and Value Differences Patient and Community Focus and Collaborate RESPONSIBILITIES: Reviews and triages telephone encounters, calls, and messages from patients and de-escalates or escalates to the appropriate care team or clinicianPrioritizes urgent medical triage to patientsProvidestimely assessment for smart scheduling into clinician scheduleReviews lab results, imaging results, consultation notes, and clinical summaries and identifies those that need to be directed to the respective clinicians for reviewEffectively communicates with appropriate disciplines involved in the care of patients, and provides information and recommendations to providersDevelops professional working relationships with health care providers and facilities, generating an open flow of information and support of home care goalsPromotes effective working relationships with nursing personnel, the management team and health care providersDetermines individual patient needs for services, referrals and training and provides patient education as necessary on plan of careAttends required meetings and huddlesSeeks guidance and resources, when necessary, in care and treatment of patientsProvides guidance and direction to colleagues and acts as a resource/mentor for nonclinical staffCoordinates team based care in the absence of the Program ManagerComplies with federal and local confidentiality laws, including HIPAA, ensuring patient privacyAdheres to Brown Health Medical Group Primary Care guidelines and policies for protecting patients’ demographic, clinical and financial informationPerforms other job-related duties as assigned MINIMUM QUALIFICATIONS: Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing.Education and ExperienceGraduate of an accredited school of nursingCurrent Rhode Island RN license in good standing3-5 years of appropriate nursing experienceKnowledge, Skills, and AbilityCapacity to collaborate with colleagues and teammates as necessary to ensure quality workAbility to work both on a team and independently,with the capability to self-directAbility to use technology with proficiency in electronic medical record (EMR) platforms and Microsoft Office applications such as: Outlook, Excel, and Word116457852 class=has-tooltipAbility to function well in a high-paced and at times stressful environmentCapacity to multi-task, think critically and problem solveClear assessment, creativity, judgment, and decision-making skills116457335 class=has-tooltipAbility to take initiative, organize, prioritize, andfollow through with work assignments assuring fulfillment of plans and goalsExcellent communication skills – both verbal and writtenAbility to maintain quality, safety, and/or infection control standardsAbility to perform standard clinical procedures according to established protocolsAbility to maintain professionalism andprovide superior customer service to allPhysical RequirementsThis role works primarily in a remote capacity and has daily communications through virtual meetings, some time in the office may be required for meetings, trainings, etc.

Pay Range:

$74,256.00-$148,512.00
