The Wilson County Health Department is seeking an Advance Practice Provider II to join our team and provide quality, compassionate healthcare to our community.

General Clinic
The position works Monday through Friday, with the Health Department closed on major holidays.

A. Clinical Services: 90%
1. Family Planning:
APP performs appropriate medical screening procedures including a complete medical, social, and obstetrical history, a focused physical and pelvic examination. The pelvic examination includes a speculum and bimanual examination. Materials are obtained for STD investigation, cytological evaluations, wet mounts, and virology studies. Based on medical history, laboratory findings and physical examination, the APP implements treatment regimens for patients. This includes but is not limited to condoms, LARC’s, prescribing oral contraceptives, Depo-Provera injections, and other specific medications according to policies, procedures, and medical directives. The APP consults or refers patients with abnormal conditions or other problems to the supervising physician, private physician, or emergency department. The APP records findings, recommendations and treatments ordered on the patient’s record. The APP provides counseling, instruction and advice on family planning methods and other medical problems and/or medicine prescribed to the patient. The patient is given an opportunity to ask questions regarding usage of family planning methods or treatment prescribed. The APP participates in the team approach to patient care, being cognizant of the needs of all disciplines and personnel in the clinic setting.
2. Maternity:
The APP functions in the maternity clinic obtaining complete medical, social, and obstetrical history. APP sees patients at regular intervals for routine prenatal care including interim history and physical examination. This includes checking fetal heart tones, fundal height, fetal position, edema, DTR’S and clonus, weight, and blood pressure. APP provides pelvic examination, as needed, for cytology, GC and Chlamydia, Group Beta Strep cultures, wet mounts, or other tests as needed. Laboratory studies may include urinalysis, hemoglobin, rubella titer, blood type and Rh, sickle cell and other blood tests as indicated. APP refers high-risk patients to OB/GYN Medical Director for medical conditions that need closer monitoring or for a level of care that is beyond APP’s scope. APP reviews glucose screen results. APP consults and/or refers patients with abnormal conditions to the medical consultant for O.B. The APP functions in the clinic with other members of the maternal health team.
3. Adult Primary Care:
The APP functions in this clinic by obtaining complete medical histories and performing complete physical examinations to include speculum and bimanual on female patients. Samples are obtained for STD investigation, wet mounts, and cytological evaluation. Hemoccults are also performed as indicated. Laboratory work includes a full battery of tests used to diagnose and treat medical problems that are present in the primary care area as well as routine blood tests such as A1Cs, hematocrits and hemoglobins, cholesterol and screening for glucose. Other blood tests such as thyroid and lipid panels are performed as indicated. Routine urinalysis and pregnancy tests are done as needed. The APP may prescribe and/or treat health problems. Position consults with supervising physician and/or refers patients as appropriate.
Job Description –Advanced Practice Provider
4. General Clinic:
The APP sees patients presenting with a variety of complaints. APP sees patients that come in to be evaluated for sexually transmitted diseases, breast masses or other breast problems and those patients’ needing evaluation for pregnancy. The APP is also consulted by the clinic nurses regarding patients with hypertension, diabetes, urinary tract infection, upper or lower respiratory infection and other medical problems. APP obtains appropriate medical and social histories, depending on the requirements of each patient. Physical examinations may include heart and lung exams, ear, nose, throat, and abdominal exams. Pelvic exams including speculum and bimanual examination are performed on female patients. Samples are obtained for STD investigation, cytology, wet mounts, and urology studies. The APP obtains samples for STD investigation and virology studies, if needed, on patients. Based on physical and laboratory findings, patients may be referred to private M.D. Treatment is administered according to policies, procedures, and medical directives. Examples of these are Monilia, Trichomoniasis, Haemophilus, and Pediculosis pubis, known GC, Chlamydia, and Syphilis contacts.
5. Child Health:
The APP works in child health to obtain medical, social, and prenatal histories and perform routine periodic physical examinations. Also, incorporated in this clinic are immunizations, dental varnishing and a variety of labs and screenings. Labs and screenings may include tuberculin testing, hemoglobin, sickle cell testing, lead level, routine urine, blood pressure, speech and hearing testing, vision screening, developmental screening, counseling, and anticipatory guidance to include accident prevention and safety, nutrition counseling, dental screening and referral as needed. The APP may prescribe and/or treat health problems. Position consults with supervising physician and/or refers patients as appropriate.
B. Child Fatality Team Provider: 5%
1. Serves as the Provider representative on the Child Fatality Team.
2. Reviews medical records of children being reviewed by the team prior to the
scheduled quarterly meeting and be prepared to discuss findings with the team.
3. Collaborates with team members in identifying deficiencies in the delivery of
services to children and/or families and to make recommendations that would
improve the outcomes of children and families served by Wilson County.
C. Other Duties: 5%
1. Oversees the flow/structure of clinic and makes recommendations to Director of
Nursing and Nursing Supervisor as needed.
3. Assigned duties related to Bioterrorism and Emergency Response activities, to
include staffing of emergency shelters.
4. Administrative duties related to clinical services including chart audits/reviews
and mandatory trainings.
5. Duties as assigned by the Health Director and/or their designee.
Family Nurse Practitioner: Licensed as a Registered Nurse by the N.C. Board of Nursing, graduation from a Nurse Practitioner program approved by the N.C. Joint Subcommittee of the Board of Nursing and Board of Medical Examiners, and “approved to practice medical acts” based on education and experience by the Joint Subcommittee, and preferably one year of experience as an Advanced Practice Provider, or an equivalent combination of education and experience.
Physician Assistant: Graduation from a Physician Assistant Program accredited by ARC-PA, passed the Physician Assistant National Certifying Examination, and licensed as a Physician Assistant by the North Carolina Medical Board, and preferably one year of experience as an Advanced Practice Provider, or an equivalent combination of education and experience.
License or Certification Required by Statute or Regulation
- Employee is required to maintain a current license to practice as a Registered Nurse in North Carolina by the N.C. Board of Nursing (FNP).
- Maintain certification with National Commission on Certification of Physician Assistants (PA)
- Maintain license with the N.C. Medical Board
- Maintain a current registration with the Board of Pharmacy
- Maintain or be eligible to obtain DEA license
