Oak Valley Health is seeking a highly motivated and experienced Infection Control Practitioner (ICP) to join our dynamic Infection Prevention and Control (IPAC) Hub Team. This specialized team provides expert IPAC consultation, education, outbreak management support, and clinical crisis response to long-term care homes, retirement homes, and other community healthcare settings across the region.

The ICP will play a key role in advancing infection prevention and control practices both within Oak Valley Health and throughout the community. The successful candidate will serve as a clinical consultant, educator, and change leader, supporting healthcare organizations in strengthening their IPAC programs, mitigating infection risks, and responding effectively to outbreaks and emerging infectious disease threats.

While the position is primarily based at the Markham site, regular travel to community partner organizations and healthcare facilities is required.

Infection Prevention and Control Leadership

  • Provide expert IPAC consultation and guidance to long-term care homes, retirement homes, and other community-based healthcare settings.
  • Conduct comprehensive IPAC assessments, audits, and gap analyses to identify opportunities for improvement and ensure compliance with provincial and organizational standards.
  • Support the development, implementation, monitoring, and evaluation of infection prevention and control policies, procedures, and best practices.
  • Collaborate with healthcare leaders and frontline teams to strengthen IPAC capacity and promote a culture of safety.

Outbreak Management and Clinical Crisis Support

  • Lead and support outbreak investigations and management activities in collaboration with public health authorities and healthcare partners.
  • Provide on-site and virtual support during infectious disease outbreaks and clinical crises.
  • Participate in rapid response activities, including risk assessments, containment strategies, and implementation of corrective action plans.
  • Monitor emerging infectious disease trends and recommend evidence-based interventions to minimize transmission risks.

Education and Capacity Building

  • Design, deliver, and evaluate IPAC education and training programs for healthcare workers, leadership teams, physicians, volunteers, and community partners.
  • Mentor and coach staff in the application of IPAC principles and best practices.
  • Develop educational resources, tools, and learning opportunities to support ongoing competency development.

Surveillance, Quality Improvement, and Program Evaluation

  • Conduct surveillance activities and analyze infection-related data to identify trends, risks, and improvement opportunities.
  • Support quality improvement initiatives aimed at reducing healthcare-associated infections and enhancing patient, resident, and staff safety.
  • Participate in program evaluation, accreditation activities, and organizational quality initiatives.
  • Prepare reports and recommendations for leadership, stakeholders, and community partners.

Collaboration and Professional Practice

  • Work collaboratively with multidisciplinary teams including nursing, environmental services, occupational health, quality and patient safety, public health, and community healthcare providers.
  • Act as a trusted IPAC resource and consultant for internal and external stakeholders.
  • Contribute to organizational strategic priorities and initiatives
  • Maintain current knowledge of IPAC standards, legislation, best practices, and emerging evidence.

What you bring to the role:

  • Expert infection prevention and control knowledge is required to act as a resource and problem solving

  • Baccalaureate degree in a health care related field is required with at least two years experience in infection prevention and control.

  • Certification from the Certification Board of Infection Control and Epidemiology (CIC) is an asset or eligible for certification within 6 months of hire.

  • Member of the Infection Prevention and Control Canada (IPAC CANADA)

  • Possesses knowledge of epidemiological principles, statistics, microbiology and infectious diseases

  • Motivated and self-directed individual with the ability to function effectively both autonomously and as a team member

  • Demonstrates effective leadership skills

  • Well-developed communication (oral, written and presentation) and interpersonal skills

  • Excellent organizational and time management skills; able to set and adjust priorities

  • Knowledge and application of Adult Education principles

  • Knowledge of the CSA Z8000 Standards for Canadian Health Care Facilities

  • Demonstrates proficient computer skills (e.g. Microsoft Office Suite, understanding of various social media platforms, internet searches, e-mail comfortably, etc.)

  • Must be able to work on a rotating on-call schedule

  • Have the ability to travel to the Uxbridge Cottage Hospital site as required

  • Demonstrated good performance and attendance records with the ability to maintain these same standards

  • Demonstrated commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required
