Healthy At Home Caregivers is seeking an experienced Home Care Outreach & Business Development Manager to expand our referral network and strengthen our presence throughout Orange County and surrounding Southern California communities.

This is not a traditional marketing or sales position.

We're looking for a relationship-builder who understands the home care industry, enjoys being in the community, and knows how to develop trusted partnerships that lead to quality referrals and long-term growth.

What You'll Do

• Develop and maintain referral relationships with: 
  • Hospitals 
  • Discharge Planners 
  • Case Managers 
  • Social Workers 
  • Skilled Nursing Facilities 
  • Rehabilitation Centers 
  • Assisted Living Communities 
  • Independent Living Communities 
  • Physicians and Medical Groups 
  • Community Organizations 
  • Senior Centers 
  • Veterans Organizations 
  • Home Health Agencies 
  • Hospice Agencies 
  • Faith-Based Organizations 
• Represent Healthy At Home Caregivers at networking events, healthcare meetings, community events, educational presentations, and professional organizations. 
• Build and maintain strong relationships with CalOptima, Veterans Affairs, Kaiser, long-term care partners, hospitals, and other referral sources. 
• Identify and develop new referral opportunities throughout Orange County, Los Angeles County, Riverside County, San Bernardino County, and San Diego County. 
• Maintain an organized referral pipeline and document outreach activities in AxisCare or other CRM systems. 
• Work closely with leadership to identify growth opportunities and implement business development strategies. 
• Promote Healthy At Home Caregivers as the preferred provider of compassionate, high-quality home care services. 

Requirements

Required Qualifications

• Minimum 3–5 years of successful business development, outreach, or account management experience within the home care industry. 
• Home Care experience is required. Applicants from other healthcare industries without home care outreach experience will not be considered. 
• Thorough understanding of referral development and relationship management within the home care industry. 
• Knowledge of Orange County's healthcare community and referral landscape. 
• Familiarity with hospitals, physicians, discharge planners, case managers, skilled nursing facilities, senior communities, and community organizations. 
• Experience working with CalOptima, managed care organizations, Veterans Affairs, or similar healthcare programs is highly preferred. 
• Excellent communication, presentation, and networking skills. 
• Strong organizational skills with the ability to work independently. 
• Professional appearance and excellent customer service skills. 
• Valid California driver's license with a clean driving record. 

Preferred Qualifications

• Bilingual English/Spanish strongly preferred. 
• Existing relationships within the Orange County healthcare community. 
• Experience using AxisCare or similar CRM software. 
• Community education and public speaking experience. 

Compensation & Benefits

Salary: $60,000–$74,000 annually, based on experience.
